INTRODUZIONE

Care/i Colleghe/i,

con grande piacere Vi invito alla 9^ edizione del nostro Hernia-Workshop

“Umbilical hernia: more than a simple hernia?”.

La tematica potrebbe sembrare banale, ma effettivamente non lo è.
Siamo di fronte ad una patologia che può variare dall’estremamente semplice ad un quadro altamente complesso. Basti pensare alle recidive di grandi dimensioni, ai laparoceli in sede ombelicale, alle ernie ombelicali postpartum associate a diastasi dei retti.
Negli ultimi anni ho notato un incremento notevole del numero di tecniche chirurgiche proposte. Un’evoluzione che desta qualche preoccupazione in quanto l’atto chirurgico sta assumendo tonalità finalizzate più alle esigenze del chirurgo che non del paziente. Credo sia giunto il momento di fermarci un attimo per rivalutare con attenzione sia le indicazioni all´intervento, che la fisiopatologia che sta alla base della malattia. Chi ci dice che l’intervento chirurgico da solo possa portare alla risoluzione del quadro patologico?
La parete addominale sta assumendo progressivamente le caratteristiche di un’unità a sé stante. Definirla un organo è senz’altro prematuro, ma come tale dovrebbe essere vista. Certo è che, per trattarla al meglio, dobbiamo impegnarci nello studio sia della sua struttura che della sua funzione.
Inoltre si sta delineando sempre più la necessità di un modello di trattamento multidisciplinare, nel quale la chirurgia rappresenta un pilastro importante, rimanendo comunque parte di un’entità maggiore.

In questa prospettiva, il 5 ottobre 2018, Vi attendo numerosi qui a Brunico, piccola cittadina alle porte delle Dolomiti, per discuterne assieme e per confrontarci con Colleghe/i di altre nazioni.

it is my great pleasure to invite you to the 9th edition of our Hernia-Workshop

“Umbilical hernia: more than a simple hernia?”.

The topic may seem trivial, but it is actually not. We are facing a pathology that can range from an extremely simple to a highly complex pattern: just think of large recurrences, umbilical post-incisional hernia or postpartum umbilical hernia associated with rectus muscle diastasis.
In recent years, we have noticed a significant increase in the number of surgical techniques proposed and this evolution gives rise to some worries as the surgical act has transformed into something aimed more at the needs of the surgeon than at those of the patient. I deem it necessary to stop for a moment in order to carefully re-evaluate both the indications for surgical intervention and the pathophysiology that underlies the disease. Are we sure that surgery alone can resolve the clinical picture?
More and more, the abdominal wall is assuming the characteristics of a separate unit. Even if it might be premature to define it as an organ, that’s without doubt the way it should be seen. In any way, in order to treat it better we must engage in the study of both its structure and its function. Furthermore, there is an increasing need for a model of multidisciplinary treatment where surgery represents an important but not the only pillar of a larger entity.

It is in this perspective that I extend to all of you a cordial invitation to join us on October 5, 2018 here in Brunico, a small town in the Dolomites area, in order to discuss this important topic with Colleagues from other countries.
Looking forward to seeing you, I’d like to recall a famous quote by Stephen Hawking:

“remember to look up at the stars and not down at your feet. However difficult life may seem, there is always something you can do and succeed at”.
